Catheter-Based Therapy for Saphenous Vein Bypass Graft Disease.

چکیده

Improved patient survival and the limitations of repeat coronary artery surgery have generated a growing population of patients referred for catheter-based treatment of vein graft lesions. In this subject review, the current state of knowledge regarding therapeutic options for these difficult lesions is discussed. Despite improvements in technique and materials, risks of treatment with balloon angioplasty remain higher than for other lesion types. The underlying lesion pathology determines therapeutic risk to a large degree. Newer therapeutic alternatives appear to offer advantages over balloon angioplasty, but improved clinical outcomes have not been demonstrated in appropriate trials for most new devices and treatment strategies. On-going studies will clarify the utility of some new devices, and other studies are needed.

Transaortic Intra-Aortic Balloon Pump Catheter Insertion through a Separate Saphenous Vein Graft in Patients with Severe Aortoiliac Disease

BACKGROUND Intra-aortic balloon pump (IABP) is the most widely used mechanical assist device for hemodynamic support in high risk patients undergoing cardiac surgery. The aim of our study was to confirm whether transaortic route is a suitable alternative to allow IABP insertion in patients with severe aortoiliac diseases. METHODS This study included 7 consecutive patients undergoing coronary ...

A case of giant saphenous vein graft aneurysm successfully treated with catheter intervention

The patient was a 67-year-old man who had undergone coronary artery bypass graft surgery using a saphenous vein graft (SVG) 22 years before. Computed tomography angiogram revealed a large aneurysm of the SVG (38 × 42 mm in diameter; 80-mm long) and total occlusion of the left anterior descending artery (LAD). We first performed percutaneous coronary intervention for chronic total occlusion of t...

The clopidogrel after surgery for coronary artery disease (CASCADE) randomized controlled trial: clopidogrel and aspirin versus aspirin alone after coronary bypass surgery [NCT00228423]

BACKGROUND Saphenous vein graft disease remains a major limitation of coronary artery bypass graft surgery. The process of saphenous vein intimal hyperplasia begins just days after surgical revascularization, setting the stage for graft atherosclerotic disease and its sequalae. Clopidogrel improves outcomes in patients with atherosclerotic disease, and is effective at reducing intimal hyperplas...

Correlation of platelet survival time with occlusion of saphenous vein aorto-coronary bypass grafts.

Platelet survival time is frequently shortened in patients with coronary artery disease, and it is one of several factors that might contribute to graft occlusion after saphenous vein coronary artery bypass (CAB). In 35 patients with CAB, average platelet survival (autologous labeling with 51Chromium) was shortened in 20 with one or more saphenous vein grafts occluded and normal in 15 with all ...
